Services for Developmentally Disabled

Our service coordinators have both the education and experience in working with people who have Developmental Disabilities and their families. We assist those persons and their families to set goals for themselves and then help work towards them. Service coordinators lend support in finding persons can best help meet those goals. Care discussions focus on living arrangements, educational or vocational supports, transportation and other individualized interests and who can then assist in obtaining those services.

Service coordinators will help form an interdisciplinary team to help the person serviced and their family member. The team will meet, and together will decide how everyone can work towards established goals. These goals and responsibilities are written in a the Person Centered Plan.

Client Quote:

"The services I receive allow me to live on my own in my own home and assist me so I won’t have to live in a nursing home."
